## How to use the fields

- **Source and issuing organisation:** Link the original statement, notice or confirmation. A repost of the same statement is one source, not independent corroboration.
- **Document and effective/event dates:** Keep them separate. An invitation date and a meeting date answer different questions.
- **What is established:** State the narrow point supported by the material, such as an agreement signed or a planned system transition.
- **What remains open:** List the implementation status, affected service or order requirement still needing confirmation.
- **Owner and next check:** Name the person who will obtain the next record and agree when the answer will be reviewed.
